SERVICES

Jacobs Home provides Residential and Community-Based Services that are customized
to meet the needs of disabled residents:

Residential Services, including but not limited to:

  • Personal Needs Assessment
  • Health and Safety Training & Supervision
  • Development of Activities of Daily Living Skills
  • Specialized Dietary Planning
  • Social Appropriateness Training
  • Therapeutic Implementation Support; including: Occupational, Physical, Speech,
    Behavioral, Recreational and other therapeutic plans.
  • Transportation Needs, in conjunction with programming.

Community - Based Services, including but not limited to:

  • Supervision & Guidance while participating in community based activities
  • Therapeutic Implementation Support, including: Occupational, Physical, Speech,
    Behavioral, Recreational and other therapeutic plans
  • Development of Community Living Skills
  • Homework Assistance

    • We meet with families to ensure individual goals are set based on education, social and Activities of Daily Living needs.
